چکیده

Neglected or chronic ruptures of the patellar tendon are defined as ruptures presenting after 6 weeks. Late presentation is often due to neglect or missed diagnosis. With chronic ruptures, the patient presents with weakness, instability, and pain. Management of patients with chronic ruptures is difficult due to adhesions, proximal patellar migration, contracture, and quadriceps muscle atrophy. Before reconstruction of the patellar tendon (using autografts, allografts, or synthetic materials), the position of the patella must be normalized. Several methods had been reported to relocate the proximally migrated patella distally to its anatomic location, including preoperative traction, intraoperative traction, quadricepsplasty, and external fixation. The aim of this case report was to present the use of ring and wire external fixation applying the lengthening principles of Ilizarov to mobilize the proximally migrated patella distally as a successful treatment of chronic neglected patellar tendon rupture followed by second-stage patellar tendon reconstruction using an allograft. We report our experience in 2 cases, with an improved modification of the external fixator construct used for distal patellar transport in case 2. This report of 2 cases was approved by the Hospital for Special Surgery Institution Review Board.

Surgical Technique to Bring Down the Patellar Height and to Reconstruct the Tendon for Chronic Patellar Tendon Rupture

Although surgical treatment is the gold standard for chronic patellar tendon rupture, the technique of patellar tendon reconstruction is still difficult. Basically, good clinical results of surgical repair for acute patellar tendon rupture have been reported. However, the results of reconstructive surgery for chronic patellar tendon rupture are still inconsistent. Some surgical options have bee...

Late reconstruction of the patellar tendon in rheumatoid arthritis using bone-patellar tendon-bone allograft

INTRODUCTION chronic patellar tendon rupture is rare and its incidence and prevalence are unknown. Furthermore, the data about late reconstruction of the patellar tendon in rheumatoid arthritis is limited. Such condition, if not managed properly, can substantially affect the patient's activities of daily living. CASE PRESENTATION We report a case of chronic patellar tendon rupture in a 49 yea...

Neglected Patellar Tendon Rupture Treated by Trans-Osseous Encirclage Wire and Turn-O-Flap: Case Report

INTRODUCTION Although patellar tendon injuries are common but chronic patellar tendon rupture are rare injuries but severely disabling. Problems associated with this injury are scar formation, atrophy and contracture of the quadriceps, poor quality of remaining reabsorbed tendon, inadequate soft tissue, proximal patellar migration and prolonged post operative immobilization. Hence these injurie...
